in-home assessment

We conduct comprehensive home functional assessments to address barriers faced by individuals of all ages and injuries/illnesses.  We consider your physical, cognitive, and psychosocial well-being as they relate to your environment.  We will provide you with solutions for maintaining safety and independence in your everyday activities.

Everything we do is based on achieving your goals.

Recommendations may include specialized equipment, adapting ways of performing activities, or changing the environment.  

We can work with your contractor/architect for home renovations to meet the needs of you or a family member to ensure home safety.

 

Wheelchair Seating and Mobility

We provide assessment and prescription of seating, positioning, and pressure management.  Our comprehensive assessment will provide recommendations on wheelchair/power mobility based on your needs, function, and environment. 

 

HOSPITAL DISCHARGE PLANNING

We communicate with medical and rehab teams at the hospital to ensure a smooth transition back to the community. We will recommend and help arrange adaptive equipment and home supports to facilitate a safe return home to optimize your recovery.

 

Case Management (ICBC)

We act like case managers by collaborating with individuals and funders to coordinate and implement all of the necessary rehabilitation services and supports to enhance your recovery and rehab. We can get involved in your recovery at any point, including while in the hospital in acute care or after you have been injured for months to years if you are having difficulty with your recovery.

 

Return to Work

We can assist with the return to work process. We work together with your employer and other health care professionals involved to ensure a safe and successful return to work.  Example of services includes an assessment of your abilities and limitations,  job site visit, ergonomic assessment, gradual return to work planning and monitoring, and job coaching.

 

Coping & Self-management strategies

We will educate and provide you with “tools” to add to your toolbox to help manage challenging thoughts, feelings, and symptoms. We will work collaboratively to develop a plan to address these challenges through education and reinforcement of self-management strategies to improve your health and well-being. Our goal is to empower you to apply the skills learned to manage your own symptoms, so you are able to do what you need and want to do every day. Education and strategies for self-management may include:

  • Activation Planning
    • We work collaboratively with you and liaise with other health care professionals to increase your participation at home, work, and community. We help to build a daily routine with structured activities progressively and systematically.
  • Exposure Therapy
    • We use exposure therapy to facilitate recovery from a traumatic experience by re-engaging in activities that you need or want to do. We have training in using a step-by-step approach to teach new skills needed to overcome barriers to participating in your everyday activities.
  • Anxiety/Stress Management
    • We assist to develop various anxiety/stress management strategies and enable you to apply them to improve your overall health and well-being.  

 

office ergonomic assessment

We will assess your workspace and ensure it fits you and the job you are doing. We will focus on prevention and/or identify specific injuries to help lower the stress and injury by reducing poor posture/positions.  Office workers often sit in the same position for most of the day and most injuries happen at work caused by physical stress and strain, making repetitive movements and overuse.  Common symptoms include pain in your back, neck, shoulders arms, wrists, and fingers.  Repetitive injuries can lead to long-term health problems such as chronic pain and potentially decrease productivity and well-being.

 

We offer the following services:

    • Comprehensive Office Ergonomic Assessment – No Report 
      • Identify pre-existing injuries and current concerns, review work tasks and positions;
      •  Assessment of the workstation and adjust the current office equipment in place (ie. keyboard, mouse, monitor set-up);
      • Provide recommendations for improvement and ergonomic equipment
      • Education on proper postures, stretches/exercises, and strategies to prevent repetitive strain injuries.

 

    • Comprehensive Office Ergonomic Assessment – With a Report
      • All of the above with a detailed report which includes the assessment, recommendations for changes, and product recommendations.

 

    • Follow-up
      • It is recommended that you schedule a follow-up to ensure that your new office equipment has been adjusted to fit you, your workspace, and your work tasks.  We will ensure it will meet your needs to optimize your health.  

 

STROKE REHABILITATION

We work with individuals with a neurological condition to maximize their independence with all aspects of daily living. We will conduct a comprehensive assessment and provide recommendations to improve and optimize function.  This may include working on improving strength, range of motion, improve fine motor skills, address issues with memory and thinking, and more to help the individual return to his/her everyday activities.
